Case Report: Sudden Unexpected Torsades in Epilepsy

Introduction: Sudden unexpected death in epilepsy (SUDEP) is the leading cause of mortality in people with uncontrolled seizures. Every year, 1 in 1000 adults with epilepsy die of SUDEP, most commonly associated with generalized tonic-clonic seizures (GTCS). Though pathophysiology of SUDEP remains unclear, the literature suggests that cardiac arrhythmia is a leading factor in the process. Reported arrhythmias during or after seizures include asystole, bradycardia, atrioventricular-conduction block, atrial flutter or fibrillation, and ventricular fibrillation. We report an unusual case of sudden-onset Torsades de Pointes (TdP) arrhythmia followed by ventricular fibrillation in a middle-aged woman with recurrent GTCS.
